3. Original of completed Application to sponsor and Undertaking (IMM 1344A), signed by me
4. Photocopy of the completed Sponsorship Agreement (IMM 1344B), signed by me and her
5. Original of completed Sponsorship Evaluation (IMM 5481)
6. Original of completed Use of a Representative (IMM 5476)
7. Copy 2 properly completed of the Receipt I printed from the Web site
8. Statutory Declaration of Common-Law Union (IMM5409)
9. Original of completed Sponsor Questionnaire (IMM 5540)
10. Original of Application for Permanent Residence (IMM 0008 Generic).
11. Original of completed Schedule 1 - Background / Declaration (IMM 0008 Schedule 1)
12. Original of completed Spouse/Partner Questionnaire (IMM 5490)
13. Proof that I intend to live in Canada with my common-law partner once she becomes a permanent resident of Canada
14. Original of completed Additional Family Information (IMM 5406)
15. Original of completed Use of a Representative (IMM 5476)
16. Photocopy of my Canadian passport (pages showing passport number, date of issue and expiration, photo, name, surname, place and date of birth of holder).
17. An original of a letter from my current employer stating my period of employment, salary, and hours per month.
18. Photocopies of all pages of the household register ("hukou")
19. Her notarized Certificate of Birth
20. Copies of her ID and travel passports (one for travel overseas, and one for travel to Hong Kong and Macau)
21. Originalnotarized certificate of “Not Having Criminal Record”
22. Copy 2 of the Medical Report-Section A that the Designated Medical Practitioner (DMP) signed and gave to her after she underwent the medical examination
23. Five (5) photos of her taken within the last six (6) months (all color)
24.Eight (8) address labelswith the mailing address in both Chinese characters and English
Green Envelope Part A (Proof of Relationship to Sponsor):
1. Rental Agreement (5 separate leases)
2. Joint Credit Card statement
3. Joint Bank Account statement
4. Cell Phone bill showing our address
5. Our emails to each other over the years (mostly when one of us was travelling)
6. Envelopes showing our address as well as showing co-habitation
7. Photos of us taken in China on several occasions (local)
8. Two letters from our two good friends proving our relationship
Green Envelope Part B (Proof of Relationship to Sponsor):
Content: Seven (7) Folders of seven (7) trips we took together
They are chronologically ordered and include: photos, boarding passes, passport stamp records, e-tickets, hotel reservations, airport shuttle reservations, etc.
